Getting ready for Kindergarten –
FREE health and dental screenings available
through Hearts & Lives
If you, or anyone, you know have a child that will be entering Kindergarten this Fall, and does not have health or dental coverage, help is available.
Please schedule an appointment with a case worker to receive a referral letter to Mountains Community Hospital’s Rural Clinic.
Before your child can be enrolled for the 2010-2011 school year, an enrollment packet requires complete physical evaluation record dated on or after March 1, 2010, immunizations, and a dental health exam.
