About First 5 San Bernardino: The Children and Families Commission for San Bernardino County (First 5 San Bernardino) was created in order to realize the benefits of Proposition 10 (California Children and Families Act) for the County's youngest residents and their families. The act created a program for the purpose of promoting, supporting, and improving the early development of children from the prenatal stage to five years of age. First 5 San Bernardino, in partnership with 76 contracted service agencies, provides a variety of programs and services designed to promote, support and enhance the early development of children prenatal through age five in San Bernardino County.

Hearts & Lives provides referrals to help with early screening, assessment, and treatment.  Early intervention can provide families with the information and support they need to maximize their child's overall development.

Prenatal Care
Are you or someone in your family pregnant? 
We want to work with you to insure you have a healthy baby.  We can help you locate and receive the prenatal care you need.

Why:       First 5’s mission is to promote, support and enhance the early development of children, prenatal through five years of age.  We want to work with the community to explore the best ways to meet the needs of our children.

Who:       Parents, service providers, community leaders and other individuals interested in improving the lives of young children in our County.

Where
:   Based on responses to this announcement, we hope to hold at least one community conversation in each region of the County as listed below.

What:
      As we continue to address the priority areas identified in the First 5 Strategic Plan, the focus areas and related outcomes we are seeking to achieve include, but are not limited to the following:

Special Needs
–
  • Improved screening, assessment, referral and treatment of children for special needs identification and intervention
  • Improved parental ability to access and coordinate needed services for their children
Prenatal Care –
  • Increased utilization of prenatal care
  • Reduced number of pregnant women abusing substances
  • Decreased number of households in which an adult smokes
  • Improved parental ability to access and coordinate needed services for their children
Health Care Access –
  • Increased utilization of prenatal care
  • Increased number of children with health insurance and an ongoing relationship with health provider
  • Improved parental ability to access and coordinate needed services for their children
  • Increased number of parents trained in child related safety and put this knowledge into practice

Did You Know?

The first three years of life are the most critical for a child’s development. Giving your children the guidance and support they need to group up healthy involves all the skills of parenthood, nurturing, guiding, protecting, sharing and serving as an example. Rim Communities Resource Network will assist you in developing these skills and help link you to the necessary resources such as food, clothing, furniture, parent education, parent support groups and counseling, and immunizations for your children.

We want to help your family access social and medical services that promote normal childhood growth and development. Our program is available for your family and assists with referrals for the five key elements.
  • Meeting your family’s basic needs such as food, clothing and safety.
  • Assessing your child’s development at various ages.
  • Positive parenting and discipline.
  • Ensuring timely immunizations and health care for your children.
  • Helping you develop a family support system.

Requirements
  • Low income or at-risk family
  • Have a child between 0 – 5 in the family
  • Rim Communities residents (Crestline thru Green Valley Lake)
