CASE MANAGEMENT SESSION: Case management sessions are provided to ongoing clients, clients identified as core recipients of service. This could include assistance with basic family needs (food, clothes, housing) provided on-site as well as additional support to help a family move towards self-sufficiency
REFERRAL/LINKAGE SESSION Providing parents and families with connections or access to critical services and programs not represented physically at Hearts & Lives. These may include basic needs such as food, clothes, and housing assistance.
HEALTH INSURANCE ASSISTANCE This includes health insurance enrollment resources which provide, programs that ensure use of health services, strategies to retain health insurance, and insurance premium payments or subsidies.
MENTAL HEALTH/THERAPY SESSION (Done in partnership with Mountains Community Hospital and Rim Family Services) Mental health services including therapy or counseling for both adults as well as children PARENTING EDUCATION SESS/WORKSHOPS/CLASS (Done in partnership with Rim Family Services) This includes any form of group parenting education which focuses on the parent/caregiver. In parenting education, parents receive instruction on how to increase their knowledge and skills related to parenting and improved knowledge and skills related to parenting and improved family functioning.
